Responsibilities
Provide Direct Patient Care: Administer treatments and medications as prescribed, develop individualized care plans, and offer compassionate guidance to patients and families
Make a Real Difference: Complete comprehensive assessments, evaluate patient conditions, and ensure care plans are aligned with evolving needs to ensure comfort and dignity at every stage
Collaborate with an Expert Team: Work closely with physicians, social workers, and other healthcare professionals to ensure optimal patient outcomes and provide holistic, patient-centered care
Coordinate Care: Lead efforts as a case manager for your assigned patients, ensuring seamless, high-quality care across all aspects of the patient’s journey
Teach & Empower Families: Provide education on self-care techniques, symptom management, and how to ensure patients are as comfortable and independent as possible
Qualification
Required
Registered Nurse (RN) with a valid license in your state
Experience in Hospice or Home Health Care: Minimum of 1 year of professional nursing experience; hospice experience is required
Compassionate & Empathetic: A heart for helping others, especially those facing end-of-life challenges
Strong Communication Skills: Ability to coordinate care with physicians, patients, and families, and clearly document clinical notes
Graduate from an accredited nursing school (BSN preferred)
Valid RN license in the practice state
Active BLS certification
A licensed driver with an insured automobile
Preferred
BSN Preferred: Experience or education in the aged, chronically ill, restorative, or psychiatric nursing preferred
