They are seeking Advanced Practice Provider Fellows to rotate through various ICU environments, providing high-acuity and critical care to complex patients under the guidance of designated preceptors.

Responsibilities

Independently, and in close collaboration with the primary preceptor, attending physician(s), fellows, and interdisciplinary team members, provides comprehensive delivery of high-acuity and critical care to complex, critically ill patients across the continuum of care
Performs rapid, comprehensive history taking and physical assessment, including advanced hemodynamic, neurologic, and ventilatory evaluation in unstable patients
Accurately documents findings, clearly differentiating pertinent positive and negative results, abnormal trends, and critical values in the electronic health record
Orders, interprets, and acts upon diagnostic laboratory tests, imaging studies, point-of-care ultrasound (POCUS), and advanced monitoring data (e.g., continuous EEG, ICP monitoring, pulmonary artery catheterization, etc.) in accordance with evidence-based protocols
Formulates differential and working medical diagnoses using advanced clinical reasoning, integrating history, physical examination, diagnostic data, and knowledge of critical illness pathophysiology
Develops, implements, continuously evaluates, and modifies individualized, evidence-based treatment plans in dynamic critical care environments
Prescribes and titrates pharmacologic agents (including vasoactive infusions / vasopressors, sedatives, analgesics, paralytics, anticoagulants, and antimicrobial therapy) and non-pharmacologic interventions (mechanical ventilation strategies, CRRT, ECMO management, IABP, Impella, etc.) within scope of practice and institutional protocols
Initiates urgent or emergent specialty consultations and facilitates rapid transfer to higher levels of care when indicated
Leads or co-develops, implements, evaluates, and/or utilizes unit-specific clinical pathways, order sets, and evidence-based protocols to standardize and optimize critical care delivery
Actively collaborates with attending physicians, fellows, residents, nursing, pharmacy, respiratory therapy, and ancillary services regarding admissions, transfers, downgrades, and inter-facility transport to ensure safe and efficient patient flow
Recognizes and responds immediately to acute decompensation or life-threatening emergencies; independently initiates advanced life-support interventions (airway management, defibrillation, massive transfusion protocol, RSI, bronchoscopy, central/arterial line placement, chest tube insertion, etc.) while simultaneously activating emergency response protocols and alerting the primary preceptor and interdisciplinary team
Incorporates age-specific, culturally sensitive, and developmental considerations into all assessments, plans of care, and family communication
Serves as a patient and family advocate, ensuring informed decision-making, education regarding critical illness, treatment options, goals of care, palliative/extubation discussions, and ethical issues while respecting autonomy, diversity, and individual values
Provides expert consultation in acute and critical care management to other services, units, and external facilities
Maintains strict confidentiality and security of protected health information in accordance with HIPAA and institutional policy
Ensures compliance with national clinical standards, quality metrics, and regulatory requirements (TJC, CMS, SCCM guidelines, etc.) through interdisciplinary collaboration and performance improvement initiatives
Clearly explains the necessity, preparation, risks, benefits, and anticipated effects of diagnostic and therapeutic procedures to patients, families, healthcare team members, and learners
Performs advanced diagnostic and therapeutic procedures (as privileged and after demonstrated competency) including but not limited to: endotracheal intubation, central venous and arterial catheterization, chest tube placement, pulmonary artery catheter placement, bronchoscopy, thoracentesis, paracentesis, lumbar puncture, point-of-care ultrasound (eFAST, cardiac, lung), and epicardial pacing—initially under direct supervision of the primary preceptor and progressing to independent practice as competency is validated
Meets or exceeds all defined clinical milestones by the conclusion of the fellowship year, as evaluated through multisource feedback, portfolio review, and direct observation
Serves as an exemplary professional role model for colleagues, interdisciplinary team members, the institution, and the broader healthcare community through consistent demonstration of integrity, accountability, and clinical excellence
Exhibits unwavering respect, cultural humility, and collaborative spirit in all interactions with patients, families, staff, learners, and the interdisciplinary care team
Demonstrates emerging leadership by actively mentoring junior APPs, residents, nurses, and students; fostering interdisciplinary teamwork; and championing initiatives that empower others to deliver high-quality critical care
Advocates for the expanded role and value of Advanced Practice Providers in critical care through patient/family education, professional presentations, community outreach, and/or participation in hospital committees or professional organizations
Assumes progressive leadership responsibilities within the fellowship, including (but not limited to) leading quality-improvement efforts, presenting in formal educational sessions, or representing the APP group in departmental initiatives
Meets or exceeds all defined leadership and professionalism competency milestones by the conclusion of the fellowship year, as evaluated through multisource feedback, portfolio review, and direct observation

Required

Adult Acute Care Nurse Practitioner or Physician Assistant
Master's Degree as an Advance Practice Provider (Nurse Practitioner or Physician Assistant)
Less than 1 year of experience in desired specialty
License or license eligible as an Advance Practice Provider (Physician Assistant or Adult Acute Care Nurse Practitioner) as set forth by the state of Virginia for Critical Care
Must demonstrate and maintain all credentialing and licensing requirements of the UVA Medical Center, the Virginia Board of Medicine or the Virginia Board of Nursing, and the appropriate certifying bodies
Must be enrolled with all payers through Delegated Credentialing and can participate in Medicaid and Medicare for billing
Current Basic Life Support (BLS) certification through American Heart Association required
For Critical Care only, current Advanced Cardiac Life Support (ACLS) certification through American Heart Association required
Job requires standing for prolonged periods, frequently traveling/bending/stooping
Proficient communicative, auditory and visual skills
Attention to detail and ability to write legibly
Ability to lift/push/pull 20 - 50lbs
May be exposed to chemicals, blood/body fluids and infectious disease
