Advanced Practice Provider Fellow (NP/PA/APP) - Primary Care

Responsibilities
Independently and in collaboration with the primary preceptor, attending physician(s), geriatricians, urgent care physicians, and interdisciplinary team members (nursing, pharmacy, social work, care coordination, physical/occupational therapy), provides comprehensive, patient-centered clinical care in primary care clinics, geriatric assessment centers, skilled nursing facilities, home-based primary care programs, and urgent care/walk-in settings
Performs thorough, age-appropriate history and physical examinations, including functional, cognitive, mood, nutritional, fall risk, and frailty assessments, with particular emphasis on geriatric syndromes and acute undifferentiated complaints in urgent care
Accurately documents findings in the EHR, clearly identifying pertinent positives/negatives, red flags, social determinants of health, advance directives, and goals-of-care preferences
Orders, interprets, and acts on diagnostic studies relevant to primary care, geriatrics, and urgent care, including routine labs, ECGs, plain films, urinalysis, rapid strep/influenza/COVID testing, and age-specific screening tests
Formulates evidence-based diagnoses and differential diagnoses for acute, chronic, and preventive care needs across the adult-geriatric lifespan, incorporating multimorbidity, polypharmacy, and deprescribing principles
Develops, implements, and longitudinally evaluates individualized care plans that integrate chronic disease management (diabetes, hypertension, COPD, CHF, CKD, dementia, etc.), preventive health services, immunizations, Medicare wellness visits, and urgent same-day evaluation/treatment
Prescribes and manages pharmacologic therapies with attention to geriatric pharmacodynamics, renal dosing, drug-drug interactions, and deprescribing; recommends evidence-based non-pharmacologic interventions (lifestyle modification, physical therapy, fall prevention, cognitive stimulation, caregiver support)
Initiates timely specialty referrals, urgent diagnostic testing, or direct admission/ED transfer when indicated; coordinates transitions of care from hospital/SNF to home or assisted living
Recognizes and rapidly manages acute/emergent conditions commonly seen in primary and urgent care settings (chest pain, dyspnea, syncope, sepsis, dehydration, acute confusion, falls with injury, minor trauma, lacerations, infections) while activating EMS or higher level of care as needed
Incorporates age-, culture-, and health-literacy–specific considerations into every patient encounter; performs shared decision-making, motivational interviewing, and advance care planning, including POLST, Advance Directive, and/or DDNR completion
Advocates for patients and families by addressing social determinants, caregiver burden, elder abuse/neglect reporting, guardianship issues, driving safety, and access to community resources
Serves as a clinical resource and consultant for primary care, geriatrics, and urgent care topics to colleagues, nursing staff, students, and external facilities
Maintains strict confidentiality of protected health information in compliance with HIPAA
Ensures practice meets or exceeds national quality standards through interdisciplinary collaboration and performance improvement
Clearly explains the necessity, preparation, risks, benefits, and follow-up for office-based and urgent care procedures (e.g., joint injections, I&D, skin biopsy, cryotherapy, suturing, splinting, nebulizer treatments, IV fluid administration, etc.) to patients, families, staff, and learners
Performs common outpatient and urgent care procedures after demonstrated competency and privileging, including but not limited to: joint aspirations/injections, trigger point injections, incision and drainage, wound care/debridement, laceration repair, skin biopsy/excision, cryotherapy, cerumen removal, etc.—initially under direct supervision and progressing to independent practice
Meets or exceeds all defined clinical milestones by the conclusion of the fellowship year, as evaluated through multisource feedback, portfolio review, and direct observation
Serves as an exemplary professional role model for colleagues, interdisciplinary team members, the institution, and the broader healthcare community through consistent demonstration of integrity, accountability, and clinical excellence
Exhibits unwavering respect, cultural humility, and collaborative spirit in all interactions with patients, families, staff, learners, and the interdisciplinary care team
Demonstrates emerging leadership by actively mentoring junior APPs, residents, nurses, and students; fostering interdisciplinary teamwork; and championing initiatives that empower others to deliver high-quality care
Advocates for the expanded role and value of Advanced Practice Providers through patient/family education, professional presentations, community outreach, and/or participation in hospital committees or professional organizations
Assumes progressive leadership responsibilities within the fellowship, including (but not limited to) leading quality-improvement efforts, presenting in formal educational sessions, or representing the APP group in departmental initiatives
Meets or exceeds all defined leadership and professionalism competency milestones by the conclusion of the fellowship year, as evaluated through multisource feedback, portfolio review, and direct observation
Qualification
Required
Master's Degree as an Advance Practice Provider (Nurse Practitioner or Physician Assistant)
Less than 1 year of experience in desired specialty
License or license eligible as an Advance Practice Provider (Physician Assistant or Adult Acute Care Nurse Practitioner, Family Nurse Practitioner, or Adult Primary Care Nurse Practitioner) as set forth by the state of Virginia
Must demonstrate and maintain all credentialing and licensing requirements of the UVA Medical Center, the Virginia Board of Medicine or the Virginia Board of Nursing, and the appropriate certifying bodies
Must be enrolled with all payers through Delegated Credentialing and can participate in Medicaid and Medicare for billing
Current Basic Life Support (BLS) certification through American Heart Association required
For Critical Care only, current Advanced Cardiac Life Support (ACLS) certification through American Heart Association required
Job requires standing for prolonged periods, frequently traveling/bending/stooping
Proficient communicative, auditory and visual skills
Attention to detail and ability to write legibly
Ability to lift/push/pull 20 - 50lbs
May be exposed to chemicals, blood/body fluids and infectious disease
