Responsibilities

Assess patients for functional limitations and develops a therapy plan of treatment with specific goals in collaboration with the patient, family, nurse, physician and other health team members

Provide direct care to treat the medical/surgical condition through the use of accepted physical therapy modalities

Instructs and supervises the patient/family and home health aide (and others) in the home exercise program that they write and other aspects of patient care as appropriate and documents their response to instruction

Continually re-evaluates the patient’s condition and modifies the plan of treatment as appropriate

Identifies and reports significant clinical information to, physician, clinical manager, primary nurse coordinating services and other disciplines

Documents all clinical evaluations, treatments and patients response to therapy
