CASE MANAGER

The Case Manager role involves providing essential case management services, conducting assessments, advocating for clients, and maintaining clinical records to ensure effective program operations.

Responsibilities
Providing screening, referral, emergency, crisis and case management services, participating in clinical treatment team
Conducting intake assessments to determine family needs, efficiently documenting all activities, conducting activities with the client and families, providing regular input to the treatment plan
Attending community meetings to advocate for client needs, providing support and crisis intervention when necessary
Documenting clinical records and additional program specific documents while maintaining the integrity of the client
Ability to use program technology to communicate effectively with staff
On occasion, act as the authorized client transporter for necessary treatment and services
Qualification
Required
Bachelor's Degree in Social Work, Psychology or Human Services related field
Level II Background clearance- All Aspire Health Partners Internships and Careers require Level 2 clearance, with Aspire covering fingerprinting costs
Pass a Motor Vehicle Records review
