Program Manager/QDDP

Family Transitions Inc is focused on providing rehabilitative services for individuals with intellectual and developmental disabilities. The Program Manager/QDDP will oversee the management of client charts, coordinate health care services, and support the development of individual service plans to ensure clients receive appropriate care.

Responsibilities

Responsible for the day-to-day management of the client chart as outlined by the program policy and procedures
Coordinates a full range of rehabilitative and behavior management programs and health care services for individuals diagnosed with intellectual/developmental disabilities, mental health issues, and challenging behaviors
Interact with individuals in a professional manner adhering to all human rights guidelines
Support the Program Supervisor/Executive Director in the development of assessments, annual forms, discharge planning and individual service plans or treatment plans to ensure that appropriate services are provided to meet the needs of the individuals serviced
Responsible for immediately managing, implementing and monitoring person centered Individual Service plans
Responsible for facilitating new employee orientation for new Direct Support Professionals
Communicates effectively with all community and Family Transition, Inc. contacts
Completes all required clinical documentation
Follows Family Transition, Inc. Health, Safety, and Emergency guidelines
Shall be responsible for other duties as needed
Supports the Program Supervisor in the day to day operations of the Day Support Program as needed
Performs other duties as assigned
Shall be responsible for supporting the Program Director/Executive Director in the creation of clinical documentation creation such as Part V individual plans in a timely manner
Shall be responsible for the intake/admission paperwork to be completed at the time of admission into the program
Shall be responsible for supporting the Program Director/Executive completion of client quarterly reports to be completed within a timely manner due every 90 days after the admission into the program
Shall be responsible for the submission of Serious Incident reports into CHRIS within 24 hours of the serious incident
Shall also be responsible for completion of internal investigation to determine what the appropriate findings are as it relates to possible breaking of company policy or state regulations
Shall be responsible for the facilitation of staff meetings to ensure appropriate communication and guidance are provided to the staff as needed
Shall participate in administrative meetings with administrative team bi-weekly to discuss updates with paperwork and other important matters such as serious incidents and other areas of concern
Shall be responsible for other duties as needed

Required

A minimum of five years' experience working in the mental health/intellectual disability field in a combination of administrative/management, staff supervision and evaluation experience as well as direct service experience including crisis intervention is preferred
Experience in dealing with the public is required
Valid driver's license with an acceptable driving record
Effective oral and written communication
Demonstrated leadership skills in project management and the formulation of program goals and objectives and establishing a plan of action, required
Must be able to plan, manage and evaluate programs, including assessment
Must have knowledge of developmental and intellectual disabilities, classifications, effects and individual's needs
Must have knowledge of employment principles as well as best practices for programs and service delivery
Knowledge of applicable Federal, State and Local guidelines, laws and regulations, including person-centered practice, Medicaid waivers and positive behavioral supports
Family Transition, Inc. requires that all employees and contractors submit to a criminal background check and a check of the Department of Social Services (DSS) Central Registry of child abuse and neglect complaints
Each applicant for employment will be required to file a disclosure statement indicating whether they have been convicted of, or have pending criminal charges against them
Any applicant convicted of a 'barrier crime' is not eligible for employment with Family Transition, Inc. under Code of Virginia • 37.2-416
The contractors or employees are also required to complete a Tuberculosis Screening Test upon employment and every year thereafter
Contractors and employees shall have current certifications in behavior management, CPR, and first aid
