Responsibilities

Evaluating the needs of older adults

Identifying unmet needs and gaps in services

Developing person-centered care plans with older adults enrolled in the OPTIONS and Caregiver Support Program

Connecting older adults with other services and resources through resource and benefit counseling

Initiating referrals for appropriate services and supports based on need and consumer directed choice

Qualification

Aging Care ManagementNeeds AssessmentCare Plan DevelopmentHuman Services ExperienceCompassionate CommunicationOrganizational Skills

Required

Successful completion of the County Social Casework Intern program; or

Six months as an Aging Care Manager 1 or a County Caseworker 1; or

Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or

An equivalent combination of experience and training

You must meet the PA residency requirement

Hiring preference for this vacancy will be given to candidates who live within Allegheny County
