Responsibilities
Care Coordination: Lead a multidisciplinary team (social workers, dietitians, pharmacists, clinical staff, medical directors) to meet member needs telephonically or digitally
Case Management Process: Assess, develop, implement, monitor, and evaluate individualized care plans
Member Engagement: Address barriers, identify gaps, and promote improved health outcomes
Resource Arrangement: Connect members to community resources, mental health, substance abuse, financial support, and disease-specific services
Education & Advocacy: Provide health literacy, self-management, medication, and nutrition education; advocate for members
Transition Support: Prepare members for discharge or transfer, ensuring continuity of care
Documentation: Maintain accurate records of interactions and care plans
Professional Standards: Meet quality and production goals; maintain CCM certification
Qualification
Required
Nursing Diploma or Associate's degree required; BSN strongly preferred
3 years clinical nursing experience (acute/post-acute/community)
CCM certification required (or within 18 months)
Critical thinking, problem-solving, and decisiveness
Empathy and strong listening skills
Motivational interviewing proficiency
Time management and organizational skills
Intermediate computer skills (Microsoft Suite)
Ability to work independently and in teams
Excellent written and verbal communication
Preferred
1 year case management in managed care preferred
Telephonic/digital patient management experience preferred
CCP certification preferred
