NEOGOV is seeking a Case Manager (Community Navigator) to enhance community relationships with health and social service partners. Responsibilities

Coordinating services with community resources and agencies to address resident needs such as food insecurity, housing, utilities, and other emerging social issues
Maintaining detailed and timely resident records, including case notes, progress updates, and key service activities
Monitoring participant progress and follow up on referrals through virtual or in-person outreach to identify resource needs, connect residents to appropriate services, and evaluate the effectiveness of interventions to ensure needs are met
Collaborating with partners and programs to expand mobile senior services and community access initiatives, maintaining up-to-date knowledge of community resources to support effective referrals
Communicating and advocate with internal and external partners to identify and resolve community needs
Delivering education and outreach to engage residents, organizations, and stakeholders through One Stop for Help programs and related initiatives
Upholding ethical standards, cultural competency, and professional boundaries to provide excellent customer service and maintain sensitive interactions
Supporting part-time Warming Station attendants and assist with rotating station support, including assistance with delivery and offloading supplies. (e.g., cots, water, snacks, cot covers, blankets) using county vehicles
Collecting intake files and engage with residents on-site to ensure needs are met and documentation is complete
Conducting site visits to encampments where initial contact has been established by a Police Officer, Community Representative, or local residents and safely engages with individuals or groups to share resources and offer support
Educating and providing information about programs, services, and opportunities to the public at community events, resource and job fairs, and other forums, as needed
Developing and engaging in relationship building through outreach with groups, agencies, organizations, and resident-related interactions
Working onsite at partner locations and gauging resident needs with One Stop for Help operational assistance and resource sharing

Qualification

Required

Bachelor's Degree in Social Work or related field
One year of experience in case management
Valid driver's license
CPR (within six months of employment)
First Aid (within six months of employment)

Preferred

Bachelor's Degree in Social Services, Social Work, Sociology, Community Development, Public Health, Human Services, Psychology, Public Administration or Education (Adult, Special, or Early Childhood)
One year of experience in case management principles, applicable laws, rules, and regulations
