Responsibilities
Care for patients where they call home
Use your clinical skills and emotional support skills to support patients and families
Create care plans that treat the whole person—body, mind, and spirit
Work closely with providers, referral sources, and our amazing interdisciplinary team (IDT)
Be a steady, calming presence during a difficult time—and help others feel just a little more at peace
Educate families, empower caregivers, and be the go-to person when people have questions
Qualification
Required
Ability to work M-F 8:30am-5pm
On call needs as required to meet patient care needs (rotating nights, weekends, holidays)
Current Registered Nurse license in the state of Michigan
Hospice, home care, acute care, or case management experience
Confidence working independently—but also a team-first attitude
A big heart and the ability to keep calm under pressure
