New MexiCare Program Coordinator (ALTSD #10118464)
The New Mexico Aging & Long-Term Services Department is dedicated to ensuring that older adults and adults with disabilities can actively participate in their communities with dignity. The New MexiCare Program Coordinator will oversee program rules, provide technical assistance, resolve client eligibility issues, and conduct in-home assessments to support the program's mission.

Responsibilities
Oversee program rules and eligibility for New MexiCare
Provide technical assistance and support to participants, caregivers, and contractors
Resolve complex client eligibility and payment issues
Conduct in-home visits and complete assessments
Track and report for KPI's and Performance Measures, accurately and timely, as established by the agency
Assist the program with other duties as assigned
New MexiCare program oversight and support to include eligibility determinations, financial assessments, and presenting on New MexiCare programs
Tracking and monitoring of participants and caregivers cases, to include eligibility status, budget status, service planning, and follow up as needed
Provide technical assistance and support to participants, caregivers, and contractors including resolving complex client eligibility and payment issues
Conduct in-home visits and complete assessments
Track and report for KPI's and Performance Measures, accurately and timely, as established by the agency
Other duties as assigned
Qualification
Required
High School diploma or equivalent and two (2) years of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care
Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling two (2) years may substitute for the required experience
Must possess and maintain a valid driver's license and must have own mode of transportation for daily client-related travel
Personal vehicle and operation of it must follow all applicable State of New Mexico statutes and regulations
Pre-employment background investigation is required, and extensive statewide travel is required
Must pass State and Federal criminal background check
Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification within 90 days of employment
Preferred
Experience working with high volume case loads which offer guidance and direction to clients
Experience working in home and community based programs
Knowledge of Community Based Programs
Experience working with Word, Excel, and PowerPoint
Experience in writing, reviewing and implementing policies and procedures
