1+ years expTo The Rescue is looking for a full-time registered nurse case manager to help our adult patients in the comfort of their own homes. The RN Case Manager will work with a healthcare team to help patients reach and maintain their highest level of independence while ensuring that services are delivered according to medical standards and agency policies.

Responsibilities
Perform direct care skilled visits according to the physician plan of care orders
Complete comprehensive patient assessments
Develop a plan of care in coordination with the healthcare team
Communicate effectively to meet the patient’s needs
Ensure that services are delivered according to licensing guidelines, professional medical standards and agency policies and procedures
Qualification
Required
Valid driver's license and vehicle insurance
Graduate of an approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing
Preferred
One (1) year home health experience preferred
