M-F visits 3 days out of the week, with OT possible with manager approval, documentation days 2 days out of week, traveling to members homes to complete assessments/documentation

Face to Face assessments completed 3 days out of the week with 2 documentation days, completing tasks, submitting all forms/assessments within turnaround times, completing outreaches

Qualification

Required

Graduate from an Accredited School of Nursing or a Bachelor's degree and 4–6 years of related experience
