Social Services Designee/PRSC

Villa Healthcare is seeking a Social Services Designee/PRSC to join their team. The role involves providing person-centered social, emotional, and psychological support to skilled nursing residents, ensuring their needs are met and promoting their growth and development.

Responsibilities

Keeps informed about new and existing community resources
Communicates daily with staff regarding admissions and discharges to various units, facilities, hospitals, community and home
Demonstrates knowledge of regulations / standards and ability to discuss with resident and or resident representative concerning choices for advanced directives, financial power of attorney, guardianship, surrogate decision
Demonstrates competence in facility abuse prevention program. Is capable of identifying, reporting, investigating and initiating appropriate interventions according to facility policy and procedures and state and federal related laws
Documents discharge planning as an ongoing review. Initiates ongoing communication with the resident and resident’s family to assess discharge needs
Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of residents and their families
Has knowledge of the advanced directives / POLST and ability to discuss with resident, resident family or resident representative end of life care, palliative care and hospice
Is knowledgeable of resident’s financial status, diagnosis and discharge needs. Based on individual needs cooperates with Case Manager’s activities with insurance company
Is responsible for home care needs being met by the time of discharge, with a goal of arrangements completed 24 hours prior to discharge when date of discharge is known
Completes all required assessments and documentation per resident in a timely manner

Required

Ability to read, write, and communicate in English
Strong verbal and written communication skills to perform the tasks required
Must have a high school diploma or equivalent

Must be able to move intermittently throughout the workday
Must be able to push, pull move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet
May be necessary to assist in the evacuation of residents during emergency situations Limited potential for exposure to environmental hazards

Preferred

Experience working with geriatric residents is preferred
Working knowledge of criteria for Medicare, Medicaid, HMO and private insurance coverage
Knowledge of accrediting organization standards, IDPH regulations, state and federal regulations related to skilled care facilities
