Outreach and Prevention Educator (HIV Outreach Tester)
Greater Family Health is a multi-site federally qualified health center providing comprehensive medical, dental and behavioral health services within the Chicagoland area. The Outreach and Prevention Educator works to identify and provide services to individuals who are at high risk for HIV and other sexually transmitted infections (STIs).

Responsibilities
Effectively outreach to the target population, including conducting screenings, assessments, and testing
Effectively provide referrals and linkage to services for needs identified, including tracking referral compliance, and continuously working to engage the client in resources that may benefit the client
Provide counseling, education, and HIV testing to high-risk individuals through community programs, events, and partnerships and to persons that request testing
Enter required data into identified databases, as well as ensure that all documentation is consistently complete, current, and compliant with both Greater Family Health and funder requirements
Actively participate in and successfully complete initial and continuing trainings/educational sessions
Refer and support clients’ access to case management for needed resources/services
Report to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor
Engage in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers
Follow and fully implement Corporate Compliance Program
Assist staff members in other areas when needed and as directed
Performs other duties as assigned
Qualification
Required
High School Diploma or GED is required
Good writing skills
Computer literacy and ability to utilize various software, word processing and spreadsheet programs
Preferred
Bachelor of Science or Bachelor of Arts in a related field is preferred
Bilingual (English/Spanish) is preferred
Previous experience working with at risk for HIV/AIDS populations is preferred
HIV Test Counseling Certification is preferred
