Responsibilities
Work closely with the CSA team, family, and other collaterals in developing measurable family partner goals to support the family’s Individual Care Plan (ICP) and related goals
Engage caregiver(s) in activities in the home and in the community that are designed to address one or more goals/tasks on the family’s ICP
Assist caregiver(s) in meeting the needs of the youth through educating, supporting, coaching, modeling, advocating and guiding
Teach caregiver(s) how to network/link to community resources and treatment providers
Support caregiver(s) in advocating for services and resources to meet youth and family needs
Guide, support and foster empowerment including, linkages to individual, peer/parent support and self-help groups
Qualification
Required
Associate's degree in human services or related field
Must have a valid driver's license and reliable transportation
Must be a parent, guardian or caregiver of a youth with behavioral health needs
Experience in navigating child/family service systems
Must be fluent in Spanish and English
