Crisis Care Manager CSA

They are seeking a Crisis Care Manager for their Community Service Agency to support families in managing and coordinating services, particularly in crisis situations. The ideal candidate will have a strong background in child and family behavioral health, crisis management, and care coordination.

Responsibilities

Be embedded within the MCI program to focus on identifying and supporting families who are receiving CBHC MCI or intake services
Serve as key contact for the youth and family to the Eliot CSA and its services
Initiate care planning activities with all members of the family and/or caregiving system
Conduct a comprehensive home-based assessment in conjunction with other CSA Team members inclusive of the CANS, CRAFTT (for youth 12 and older), SNCD and other tools as determined necessary, which occurs in the youth’s home or another location of the family’s choice
Initiate planning that includes intake meetings, orient the family and the member to the wraparound process, identify strengths and needs of the child
Develop and implement a youth and family centered Individualized Care Plan (ICP) in collaboration with the family and collaterals
Develop an initial safety/crisis plan

Required

Bachelor's degree in human services or related field
Must have a valid driver's license and reliable transportation
Experience navigating child/family service systems
Experience working with child mental health issues and crisis management

Preferred

Eligible for 6% bilingual pay increase if fluent in needed language capacity
