Community Program Manager
Urban Health Partnerships (UHP) is seeking a Community Program Manager to join their growing team. The role involves planning, coordinating, and managing community-based initiatives and requires strong project management skills and a commitment to community-driven processes.

Responsibilities
Manage assigned projects from start to finish, ensuring workplans, timelines, milestones, and deliverables are completed timely with reliable follow-through
Coordinate and facilitate meetings, convenings, trainings, and events, including agendas and minutes, materials, logistics, and follow-up
Use digital project management tools to produce clear, accurate project plans, documentation, workflows, and outcomes that reflect both community voice and project goals
Advance multiple initiatives simultaneously without losing momentum
See the big picture and identify what’s missing
Anticipate risks, coordination gaps or delays, and address them proactively, translating insight into action
Understand when to take initiative and when to mobilize others, rallying teams, and partners toward shared goals
Support and manage cross-sectoral initiatives across UHP’s program areas, including food access, mobility, public spaces, and other community-driven efforts
Work closely with Community Liaisons, residents, and partners to maintain trust, alignment and transparency to support authentic, relationship-based engagement
Support community action planning processes and help translate community priorities into actionable strategies
Works instinctively 'in sync' with others, valuing different perspectives while aligning voices toward a common outcome, building and maintaining strong working relationships with community members, community-based organizations, and government and institutional partners
Contribute program content and coordination support for grant proposals, reports, and funder engagement
Support development and maintenance of project-related materials, social media, and web content to help uplift community stories and impact
Qualification
Required
Bachelor's degree in public health, social sciences, community development, public administration, or a related field
At least three (3) years of experience managing community-based, public-sector, or collaborative initiatives
Experience working directly with community members or community-based organizations
Strong project management, organizational, and communication skills
Ability to manage multiple priorities and work collaboratively across teams
Fluency in English required; Spanish and/or Haitian Creole preferred
Preferred
Master's degree in public health, social sciences, or a related field
Experience working with government agencies, municipalities or regional initiatives
Experience supporting or supervising community-based staff or liaisons
Experience working with digital project management tools and/or platforms
Familiarity with grant-funded projects and reporting
