Ennoble Care is a mobile primary care, palliative care, and hospice service provider. They are seeking an experienced Per Diem Nurse Practitioner to provide face-to-face assessments and care to hospice patients in the Gwinnett, GA region, working independently under the supervision of a physician.

Responsibilities

Conduct visits with hospice patients in and near the Bergen/Hudson Gwinnett, GA region

Work independently under the supervision of a physician

Complete physical exam and review of systems and assess eligibility for hospice

Speak with patients, families, and caregivers during visits and as needed

Attend IDG meetings when possible

Prescribe medications supplies and appropriate medically necessary interventions

Required

Valid ROLE certification in state of job location
