Quality Facilitator | Home Health Program

Midland Care Connection is a not-for-profit organization dedicated to helping older adults and seriously ill individuals stay independent in their homes. The Quality Facilitator is responsible for overseeing and maintaining a comprehensive Quality Assessment and Performance Improvement (QAPI) program to ensure home health services meet regulatory standards, while promoting a culture of quality and continuous improvement.

Responsibilities

Responsible for overseeing and maintaining a comprehensive Quality Assessment and Performance Improvement (QAPI) program to ensure home health services meet agency, state, and federal regulatory and licensing standards
Leading performance improvement initiatives, conducting audits and in-depth medical record reviews, analyzing quality and compliance data, and reporting findings to leadership with recommendations for corrective and preventive actions
Tracking publicly reported quality measures, adverse events, and regulatory reports, staying current with regulatory changes, and supporting value-based purchasing requirements
Providing education, training, and resources to staff, collaborating across departments, assisting with billing audits and appeals, and actively promoting a culture of quality, safety, and continuous improvement throughout the organization

Required

Registered Nurse (RN) degree with an active license in Kansas and Missouri
Knowledge and understanding of federal and state laws and regulations related to hospice, home health, home support, and palliative care programs
Strong communication skills with the ability to effectively engage staff at all levels of the organization
Demonstrated planning, analytical, and problem-solving skills
Ability to utilize technology to collect, analyze, and present quality and performance data
Valid Kansas or Missouri driver's license with current automobile liability insurance
Reliable transportation and a motor vehicle record (MVR) that meets organizational insurability standards

Preferred

Previous experience in accreditation, quality improvement, education, and/or regulatory compliance
Prior involvement with performance improvement projects, audits, or regulatory surveys
Experience supporting quality initiatives in home health or related healthcare settings
