NEOGOV is seeking a part-time and full-time licensed practical nurse (LPN) or registered nurse (RN) to join their team at Golden Age Manor, a skilled nursing facility owned by Polk County. The role involves supervising nursing personnel, planning and implementing resident care plans, and responding to emergency situations while ensuring high-quality care for residents.

Responsibilities
Supervising and evaluating performance of nursing personnel
Is designated with the authority to direct all non-professional personnel in his or her charge to act in the interests and welfare of the residents on the unit (to include schedule changes and job assignments)
Is designated with the authority to discipline all non-professional personnel in accordance with the employee manual and the union contract
Planning, developing and implementing resident care plans necessary to the individual's needs
Administering treatments, medications and diagnostic procedures as ordered by the attending physician and assigned by administration
Handling admission transfers and discharges of residents
Completing quarterly reviews, annual and other required MDS's as assigned by wing for DON
Assist in coordinating Medicare certifications
Assist with pre-admission screening
Notify resident/appropriate family members
Expected to respond to emergency situations involving the safety of residents, other employees and the facility. This included the ability to assist with a possible evacuation of residents
Assist with implementing policies/procedures with impact Medicare coverage
Other duties and responsibilities assigned by DON or ADON within the scope of Wisconsin Nurse Practice Act
Assisting staff with major nursing care problems and reporting all major problems and accidents to the Director of Nursing or Assistant Director of Nursing
Making daily rounds to assigned nursing units under assigned supervision and performing such functions as assessing residents, reviewing medications and clinical records, resident care plans and staff assignments, and, to the extent possible, accompanying physicians when they make rounds
Participates in on-site physician visits; communicates comprehensive information related to resident status and needs
Ability to plan, assign, and supervise the work of others
Ability to recognize significant changes in the condition of residents and to take necessary action
Ability to establish and maintain effective working and public relationships
Cooperating with Inservice education of nursing personnel
Notifying the attending physician when stop order policies would prohibit additional administration of medications to individual residents so there may be a decision if the order is to be renewed, and of changes in resident's conditions
Follows instructions willingly, able to meet deadlines, organizational loyalty, honesty, punctual, and dependable
Carries out supervisory responsibilities in accordance with Polk County's policies and applicable laws. These responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems
Delegates duties and care of residents appropriately
Possesses and utilizes decision-making ability
Supervises those under him/her to ensure proper policies and procedures are followed
Informally educates other as needed
Gives clear and specific direction to others
Gives timely, appropriate response to resident/nursing staff problems
Channels problems through correct chain of command
Communicated data shift to shift
Informs affected others when changes are made
Delegates and evaluates duties and responsibilities of LPN, Nurse Technicians and nursing assistants
Communicates pertinent information regarding residents care to nurses/nursing assistants
Qualification
Required
Thorough knowledge of professional nursing theory and practice
Considerable knowledge of the supervision and administration of a nursing unit in a health care facility
Considerable knowledge of restorative nursing
Considerable knowledge of psychiatric and/or geriatric nursing
Graduation from an accredited school of nursing
Other ongoing training/education as provided by facility to meet regulatory and facility policy changes
