BAYADA Hospice is hiring a compassionate and dedicated Nurse Practitioner (NP) to join their team. The NP will be an integral member of a multi-disciplinary health care team providing palliative care to clients, ensuring they receive the necessary medical care to remain at home.

Responsibilities

Making home face to face hospice recertification visits to clients in designated geographic territories

Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager

Following up with, executing, and properly documenting doctors' orders

Performing client assessments as necessary

Qualification

Nurse Practitioner licenseClinical experienceHospice experiencePalliative care experienceElectronic medical recordsComputer skillsInterpersonal skills

Required

A current license as a NP in NH

A minimum of one year of recent, verifiable clinical (medical/surgical) experience

Solid computer skills; prior experience with electronic medical records (EMR)

Ability to travel to cases as assigned

Strong interpersonal skills

Preferred

Prior home care, hospice and/or palliative care experience strongly preferred, but not required

Prior experience with electronic medical records (EMR) preferred
