Responsibilities

Answering phones, receives all referral and intake information

Complete referral/intake forms and route them appropriately for admission approval

Assist in the scheduling of appropriately skilled person to fill the case, inputs scheduling information on the computer

Check common working files for Medicare Coverage and verification of benefit period

Perform other duties as directed

Qualification

LPN licenseEMR System experienceHome Health Care experienceCustomer service experienceComputer literacyEffective communication

Required

High school graduate or GED

LPN license

Must be computer literate and able maintain simple records

Must be able to communicate effectively orally and in writing

Must have at least 3 years of Customer service experience

Must have at least 3 years of EMR System experience

Preferred

Experience in a Home Health Care or Hospice setting
