EvergreenHealth is seeking an experienced RN to join the Mammography team. The role involves providing direct nursing care to patients, coordinating education for patients and families, and collaborating with interdisciplinary teams to implement care plans.

Responsibilities

Provides direct patient care and functions as a patient advocate

Assesses the comprehensive nursing needs of an assigned group of patients

Coordinates the patient and family/significant other education process

Continuously plans, implements, and evaluates patient care, incorporating patient and family/significant other into the process

Participates with physician partners and other interdisciplinary team members to develop and implement patient and family/significant other plan of care

Communicates effectively all relevant information (e.g., patient information, safety issues, problems with equipment, quality issues, etc.) to immediate supervisor, in timely way

Communicates and documents changes in patient condition, on a timely basis, to charge nurse/supervisor and physician

Performs other duties as assigned

Qualification

Registered Nurse LicenseBLS CertificationPatient Education SkillsCare Coordination ExperienceEPIC Charting KnowledgeBreast Navigation ExperienceCommunication SkillsMultidisciplinary Team Collaboration

Required

Graduate of a nursing program (Diploma or Associate Degree in Nursing (ADN) required

Licensed as a Registered Nurse in the State of Washington

Current Healthcare Provider BLS – American Heart Association by start date

Strong patient education and communication skills

Experience in care coordination and multidisciplinary team collaboration

Preferred

BSN or MSN

Knowledge and experience with EPIC computerized charting

2 years of clinical experience in breast health and/or oncology care navigation

Breast Navigation experience / BHCN Certification (Breast Health Clinical Navigation); or obtain w/in 2 years of start date
