Social Service Designee
Aventura Health Group is a long-term care facility, and they are seeking a Social Service Designee to support the psychosocial well-being of residents. The role involves assisting residents and families with care planning, discharge planning, and connecting them to community resources while ensuring compliance with regulations.

Responsibilities
Assist residents with social, emotional, and psychosocial needs
Advocate for resident rights, dignity, and quality of life
Support residents in adjusting to admission, room changes, and changes in health status
Participate in interdisciplinary care plan meetings
Contribute psychosocial assessments and updates to resident care plans
Monitor and document residents’ psychosocial status
Act as a liaison between residents, families, and facility staff
Provide education and support to families regarding resident care and facility processes
Address concerns and assist with conflict resolution when appropriate
Assist with discharge planning and transitions of care
Coordinate with community agencies, home health services, and other resources
Ensure safe and appropriate discharge arrangements
Maintain accurate, timely documentation in residents’ medical records
Assist in ensuring compliance with CMS, state, and facility regulations
Support grievance and complaint processes per facility policy
Provide information and referrals to community services (e.g., financial assistance, counseling, housing, transportation)
Coordinate with external agencies as needed
Qualification
Required
High school diploma or equivalent
Experience working in long-term care, healthcare, or human services
Strong communication and interpersonal skills
Knowledge of resident rights and confidentiality (HIPAA)
Preferred
Associate's or Bachelor's degree in Social Work, Human Services, Psychology, or related field
Previous experience in a skilled nursing or long-term care setting
Knowledge of Medicare, Medicaid, and long-term care regulations
