They are seeking an experienced Per Diem Nurse Practitioner to conduct face-to-face assessments for hospice patients in the Morris, Essex, and Passaic County, NJ region, working under the supervision of a physician.

Responsibilities

Conduct visits with hospice patients in and near the Morris, Essex, Passaic region

Work independently under the supervision of a physician

Complete physical exam and review of systems and assess eligibility for hospice

Speak with patients, families, and caregivers during visits and as needed

Attend IDG meetings when possible

Prescribe medications supplies and appropriate medically necessary interventions

Required

Valid ROLE certification in state of job location
