Responsibilities

Providing drug information to the Medical Center staff concerning proper drug selection, optimal dosage and route of administration, recognition and management of drug interactions and reactions, and proper handling and storage of medications
Engaging in direct and/or supportive patient care for an aging adult population, which requires knowledge, skills and consideration for specific patient conditions
Providing patient education and information on the drugs prescribed upon discharge from the hospital and upon ambulatory care clinic visits
Effectively documenting pharmaceutical care in a timely and accurate manner
Accessing appropriate patient records for proper evaluations of clinical situations and therapeutic decision making. Access includes but is not limited to VACC (Mission Act), recommending appropriate patient labs (UDS, pharmacokinetics), completing consents, accessing the State Prescription Monitoring Program (SPMP), and being available for consultation to Pain Clinics, PACT or designee. Interviews patients to obtain additional information regarding disease state/drug therapy when evaluating a clinical situation
Consulting with Hematology/Oncology for proper antineoplastic or biologic therapy/dosing. Responsible for entering appropriate and correct drug, strength, dosage form, and quantity into medication profile for all chemotherapy or biologics
Compounding chemotherapy, biologics and hazardous medications utilizing sterile IV technique according to the most recent guidelines
Compounding sterile products, both uncomplicated and complicated, hazardous and non-hazardous, to be delivered in the form of large volume, small volume, infusion syringe, ophthalmic preparations and other complex pharmaceuticals
Interviewing patients to determine a medication history, when necessary, and educates patients on the proper use and expected effects of their medications when the need arises
Consulting with providers on individualized medication treatment plans and communicates recommendations verbally or in writing to the patient's health care provider/team
Developing individualized dosing regimens based on the pharmacokinetics of the drug(s) employed, renal and hepatic function, and other pertinent parameters of the patient in question, and communicates recommendations, either verbally or in writing to the patient's health care provider/team
Dispensing medication upon computer-generated orders or written orders of providers, in accordance with dosage form requirements
Inputting and/or verifying medication information into the computer system for outpatient, unit-dose, ward stock, and intravenous admixture programs
Receiving and verifying VA Community Care prescriptions for proper eligibility. Entering properly authorized VACC orders in the VISTA package to process and fill or handles as a PADR medication as required. Contacting VACC provider if a change is required or if the PADR request is denied and advises the VACC provider of a clinically appropriate replacement
Participating in the education of VA/Texas Tech postgraduate Pharmacy Residents, undergraduate pharmacy students, and pharmacy technician students as assigned
Assisting in the training of new personnel with routine job duties
Orienting, training and/or instructing employees and students in specific tasks as assigned
Completing quality assurance audits of clinical programs, drug distribution systems, controlled substance records, and other areas of Pharmacy Service as assigned
Observing stock levels on a daily basis and assists purchasing agent with ordering by maintaining a list of items needed
Receiving controlled substances and enters into stock in accordance with VA Regulations
Identifying safety violations and reports them to the Safety Officer in the pharmacy area

Required

U.S. Citizenship
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Graduates of foreign pharmacy degree programs must provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT)
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d)
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)

Preferred

VA experience (at least one year)
Hospital or Community pharmacy experience (at least one year)
Preceptor license
IV experience (at least one year)
