Bilingual Case Manager (Community-Based Program / Alameda County)

The Bilingual Case Manager provides case management services to Kinship youth and families, developing individualized plans and coordinating resources to support stability and well-being for kinship children and youth.

Responsibilities
Provide case management and crisis intervention to assigned kinship youth/family; this includes safeguarding the dignity and protecting the rights and individuality of each kinship youth/family
Coordinate services to include intake, assessment, case plan and referrals to community service providers; this includes developing a strong base of client involvement and engagement
Maintain ongoing relationships with community service providers, and other agencies/programs to educate and advocate for the needs of youth, families and communities
Liaise with service providers to coordinate resources and services for families and to follow up with linkage to services offered
Facilitate events for children and families
Facilitate youth support groups, caregiver support groups, other trainings or workshops as assigned
Develop agency material including project flyers and other outreach materials
Participate in agency meetings and community meetings as directed
Conduct outreach and recruitment activities to the targeted population to effectively promote program services
Complete and maintain all service documentation and statistics as required
Requires driving to various locations across Alameda County to deliver services at assigned office(s), in the community or via home visits; this also includes transporting youth or families to community resources and appointments as needed (Case Manager must maintain valid California driver’s license, satisfactory driving record and personal vehicle insurance coverage)
Qualification
Required
High school diploma or equivalent is required
English/Spanish Bilingual language skills required
At least 2 years of relevant work experience (with youth, families, or social services) is required
Strong computer skills and proficiency in Microsoft programs and Google suite is required
A positive, service-oriented team player who has the ability to exercise sound judgment and work independently on a day-to-day basis
Must have strong organizational and time management skills
Proven problem-solving skills
Excellent communication skills both verbally and in writing
Demonstrated knowledge of and skills in working with separation and loss, family systems, conflict resolution, reunification process and the emotional issues of kinship families
Awareness of and sensitivity to multicultural issues
Proven competence working with culturally diverse populations
Passion and commitment to serving children and families
Must have knowledge of community resources
Able to work some evenings and weekends
Must maintain valid California driver's license, satisfactory driving record and personal vehicle insurance coverage
Preferred
Bachelor's degree in Social Work or human services related field is preferred
Non-profit and/or healthcare experience is desirable
Experience with trauma-informed care and practices is preferred
