Responsibilities
Become part of our highest skilled nursing team, helping clients as they transition home from the hospital and filling last-minute openings
Provide the initial 4-6 weeks of skilled nursing care to new admission clients
Ensure each client's needs are met according to their care plan, all while delivering personalized one-on-one nursing attention at home
Perform assessments, monitor clients’ conditions, and document observations, interventions, and evaluations
Qualification
Required
Active New Jersey LPN license in good standing
CPR Certified
Willingness to work across market service area (Bergen, Passaic, & Hudson County)
Willingness to work with all ages (Infant through Geriatric)
Willingness to work in all private duty settings (In-Home & In-School)
One year of skilled nursing experience
Hands-on tracheostomy and ventilator experience
Pediatric experience
Commitment to a minimum of two twelve-hour shifts per week
Commitment to work one weekend per month
