Cone Health is seeking a Care Management Assistant to support the care management team. The role involves ensuring the accuracy of patient and insurance information and facilitating reimbursement processes, while providing administrative support to clinical staff.

Responsibilities

Provides administrative support for social workers, filling out appropriate documentation and contacting patients to ease burden of tasks off of clinical staff and increase office-wide efficiency

Maintains up-to-date calendars for appointments, deadlines and other pertinent information to keep clinical staff aware of important dates, notifying them ahead of time

Prepares documents such as referral forms, using proper grammar and spelling to effectively communicate with other providers, both in and outside the organization

Corresponds with patients, caregivers, insurance companies, post-acute vendors and community agencies, under the leadership of a Case Manager or Clinical Social Worker, to ensure accuracy of information

Informs departmental leaders about potential gaps in coverage that social workers may have to note when there may be more resources needed and allowing departmental leadership to strategize on how to tackle said problem

Performs other duties as assigned
