JUVENILE PROBATION DEPARTMENT - RE-ENTRY CASE MANAGER (GRANT FUNDED)

NEOGOV is looking for a Re-entry Case Manager to support families with court-ordered youth. The role involves case management services, home visits, and developing comprehensive case plans to facilitate the reintegration of youth into their homes.

Responsibilities

Provides service referrals and linkages to the community to assist with the needs of the family while youth is in placement and upon reintegration into the home or while on community based supervision
Coordinates and/or attends home visits, phone calls, conference calls, Admission, Review, Dismissal( ARD), school meetings, facility meetings, case staffings, court hearings
Assists with the development of comprehensive case plans that align with the therapeutic objectives outlined in the youth’s treatment plan and/or case plan
Educates the youth and/or family members through individual, family and/or group sessions designed to strengthen coping, social, self-regulating, resiliency, and independent living skills as well as guide participants in their decision-making abilities through development and/or enhancement of natural supports
Provides referrals and linkages to community and/or educational services to facilitate the reintegration process
Assesses and evaluates the home environment and community to aid in the identification and development of creating or enhancing natural supports
Assists youth and family with determining eligibility for financial, social and/or educational services
Monitors services and evaluates the effectiveness of services in meeting treatment and case management needs
Attends and participates in meetings, staffing, and hearings
Provides requested information to attorneys, judges, and other departments
Collaborates with other agencies and service providers to ensure suitable services are being provided to participants and their families
Documents activities performed in accordance with standards established by Texas Juvenile Justice Department (TJJD), Texas Health and Human Services Commission (THHSC) and the department
Performs data entry for participants case files, benefits, and other information
Conducts file audits on a regular basis, requests necessary records, and updates records into appropriate systems and databases
Implements Effective Evidence Based Practices (EBP) and/or promising practices as deemed appropriate by the JPD
Substitutes, if assigned, during temporary absences by performing delegated duties sufficient to maintain continuity of normal operations
Commits self to providing excellent customer service and demonstrate commitment through cooperative team and individual efforts
Creates a high-quality work culture through participation in and emphasis on training and mentoring to develop leadership, management, and technical skills in self and all employees, including safety related training and skills
Facilitates communication and coordination of activities between members of the Clinical and Challenge teams by involving the youth and family members in the decision-making process to minimize fragmentation and/or duplication of services
Facilitates skills development services to include group, family and individual services to enhance independent living and reentry skills
Maintains contact with youth and family through phone calls, home visits and electronic communication
Coordinates contact with service provides, other staff, attorneys, and other persons associated with the youth
Attends and facilitates the interdisciplinary team meetings during case reviews, staffing phase assessments, and/or conference calls
Assists the youth, family, and interdisciplinary team members in defining emotional, physical, social, and cultural needs in the development of treatment and case plan objectives
Maintains compliance with grant funded requirements
Initiates, generates referrals for mental health, medical, community and contracted services to include e-financial and payment submissions
Performs such other related duties as may be assigned
Must be available to work non-traditional hours and be on call as necessary on official business
Work remotely as directed
Work more than 40 hours during the work week as needed
Travel by land or air to fulfill duties

Required

Bachelor's degree from an accredited college or university with a major in social, behavioral or human services related field
one (1) year experience of case management or care coordination experience
Experience working with juveniles is preferred
Bilingual (Spanish/English) preferred
any equivalent combination of experience and training which provides the required knowledge, skills, and abilities
Knowledge of: case plans aligned to treatment recommendations; services and community resources that may be utilized to facilitate the rehabilitative and reintegration process; crisis stabilization techniques and/or resources; socialization services; coping services; self-regulating; life skills and independent living groups and/or services; Departmental, TJJD, THHSC and licensing standards; English usage, grammar, punctuation and spelling; techniques for effective interpersonal communications; modern office procedures, methods and computer equipment to include Windows and Microsoft based products
Skill/Ability to: learn and apply current juvenile justice theories and practices and evidence-based programs; practices relevant to case management practices for individuals with a behavioral and/or mental health disorder; be willing to work in a pre and post adjudication secure and none-secure residential setting; possess a high degree of maturity coupled with an interest in working with children/adolescents, their families and ability to work positively and effectively with maladjusted youth; develop case plans aligned to treatment recommendations; identify services and community resources that may be utilized to facilitate the rehabilitative and reintegration process; aid in the amelioration of identified symptoms/behaviors associated with a behavioral health and/or mental health disorder; apply crisis stabilization techniques and/or resources; facilitate socialization, coping, self-regulating, life skills and independent living groups and/or services; adhere to Departmental, TJJD, THHSC and licensing standards; possess excellent communication and interpersonal skills; engage youth and family members in the therapeutic and rehabilitative processes; foster teamwork and partnerships with co-workers, departmental personnel and community stakeholders; work positively and effectively with all types of children including severely maladjusted youth; possess organizational and time management skills; record keep; ensure that services are prioritized; adhere to Departmental, State and Federal standards and laws regarding treatment records; work efficiently both independently and as part of a team; establish and maintain effective working relationships with departmental personnel, clients, other County employees and officials, clinical service providers, law enforcement, the courts and the general public; flexible to the needs of the department and program and make required changes to fulfill those needs; successfully complete the department's pre-employment requirements; pass a Criminal Background Check

Benefits

16 holidays per year as recognized.
Accruals are credited on a biweekly basis and available for use as it is accrued after 6 months of continuous service. Year to year carry-over accruals are limited to twice the employee's annual rate. Part-time employees accrue vacation at half the rate of full-time employees.
15 days (5 through less than 15 years of service)
20 days (15 years of service or over)
Accruals are credited on a biweekly basis and available for use as it is accrued after 3 months of full time service. Employees accrue 15 days of sick leave per year.
Military Leave – 15 days of paid leave per fiscal year for active military service
Bereavement Leave – 3 days of paid leave for the death of an immediate family members and 4 hours for other family members and friends.
Consumer Driven Health Plan (CDHP)
Core Plan
Retirement benefits are provided by the Texas County and District Retirement Fund (TCDRS). Regular employees become a member of TCDRS upon hire. All regular employees are required to contribute 7% to the retirement fund and the County matches 250% upon eligible retirements.
Employees may defer earnings, on a pre-tax basis, into a 457 deferred compensation program. This is an optional program.
Full-time County employees are provided a basic life insurance benefit of $10,000 life insurance at no cost. Supplemental coverage is available for additional fees.
The Long-Term disability program provides 60% of the employees' income when the employee becomes disabled because of an illness, injury or accident for a period above 90 days.
The Short-Term disability program provides 60% of the employees' income when the employee becomes disabled because of an illness, injury or accident for a period shorter than 90 days.
Employees who retire from the County are eligible to continue Health Insurance at the rate set by the El Paso County Risk Pool Board.
Employees are enrolled in an Employee Assistance Program that provides a variety of counseling services.
The section 125 program is optional for employees and provides a method to pay medical and dependent care expenses on a pre-tax basis.
Reimbursement of $15 per month for 6 months of continuous gym usage.
Reimbursement up to $2000 per calendar year for eligible education expenses.
The Employee Fitness and Wellness Center opened in October 2021 at the County Courthouse. The cost is $5 per employee per month.
