Responsibilities

Inspires staff and makes effective use of their energies, resources and talents in promoting the objectives, values and vision of the organization; Helps coordinate and disseminate information; Assists Administration in establishing and implementing agency priorities, goals and strategies; Helps facilitate and foster effective and supportive relationships between staff and Administration; Must complete all required trainings; Responsible for direct provision of case management services and coordinating all direct services; Interviews individuals and others as appropriate. Gathers personal, medical, social, educational and vocational information; Responsible for the development of an initial Individual Support Plan (ISP). Responsible to use a person centered planning approach to help the planning team develop a comprehensive ISP to meet the participant's identified needs in the least restrictive manner possible; Coordinates and assures delivery of direct services; Reassesses and modifies plan goals and objectives as needed; Ensures ongoing monitoring of services delivery included in the individual's ISP, records progress and changes; Responsible to ensure face to face meeting with each individual to ensure the individual's health and welfare, to review the individual's progress and ensure that the ISP is being implemented as written; Provides supportive services to the client and family, assisting them during the entire process of planning and delivery; Maintain current knowledge of private and public resources in the community; Ensures individual's choice of providers by providing information to ensure individuals make fully informed decisions; Ensures individuals with behavioral support plans and crisis intervention plans are consistent with the ISP; Develops working agreements with community resources; Interprets service resources to clients and serves as liaison between client and service providers; To conduct individual and family interviews and to use them to identify individual needs; To establish and maintain effective working relationships with consumers, their families, other staff outside agencies and institutions and the general public; Plan and coordinate team meetings at least annually to conduct a comprehensive review of the ISP, or more frequently as individual needs change; At least annually collect information to inform level of care reevaluations; Participates in the standardized needs assessment process to maintain development of the ISP process including any necessary ISP updates; Facilities the completion of additional assessments, based on participants' unique strengths and needs, for planning purposes and ISP development in order to address all areas of needs and the participant's strengths and preferences; Respond to and assess emergency situations and incidents and assure that appropriate actions are taken to protect the health and welfare of individuals; Monitor participant and/or family satisfaction with services; Arrange for modifications in services and service delivery, as necessary to address the needs of the participant, and modify the ISP accordingly; Ensure that services are appropriately documented in HCSIS on the ISP; Communicate the authorizations status to ISP team members as appropriate; Maintain knowledge of current social case management principles, techniques and methods; Maintain knowledge of current social, economic and health problems and resources and then recognition of their impact on the growth and development of people; Maintain knowledge of the basic principles and methods of program interpretation and community organization; Maintain knowledge of social welfare policy and law as they relate to agency function and purpose and societal structures; Provide information to participants on fair hearing rights and assist with fair hearing requests when needed upon request; Attend and participate in community activities representing The Arc Advocacy Services; Participate/Contribute in agency fundraising activities

Required

The candidate must have work experience and have an understanding of local, state and the federal special education systems
Understanding of Pennsylvania's MH/ID service systems
Current welfare, social security and other benefits systems
Has a working understanding of other agencies, community services and systems and their relationship to programs and families
Possess high quality organizational skills
Ability to work in a high pressured job situation and adhere to strict time guidelines
Excellent communication/writing skills to interface effectively with all County Personnel, Program Coordinators, Contract Coordinators, Staff Members, Contracted Therapists and other agencies and persons which may be involved in the referral process
Ability to work effectively with people and aid them to grow in the constructive use of their potential in adjusting to their specific needs
Ability to understand and accept the needs and rights of others and to work with individuals who are physically challenged, emotionally troubled, or economically disadvantaged
Ability to plan and organize work, prepare adequate records and reports, set priorities and maintain a caseload in an effective and timely manner
Current FBI, Criminal History and Child Abuse clearances will be required at time of hire
Employee must be willing to update clearances as per regulations
The employee must be legally authorized to drive a motor vehicle in the State of Pennsylvania
They must have a valid Driver's License and must have current automobile insurance
A 3-year driver record check is required at time of hire and it must be updated annually
The employee must be able and willing to drive throughout the counties that we provide services
