Program Coordinator-Augmented Multidisciplinary Team (A-MDT)
Lifespan of Greater Rochester is a trusted mission-driven, regional nonprofit dedicated to helping older adults navigate the challenges of longer life. The A-MDT Coordinator will be responsible for organizing and launching county-based Augmented Multidisciplinary Teams, facilitating meetings, and providing case consultations without direct client interaction.

Responsibilities
Conducts outreach to develop and maintain a comprehensive network of community, professional and governmental-based organizations for A-MDTs in the Finger Lakes Region
Organizes, coordinates, and facilitates regularly scheduled A-MDT meetings (in-person, virtual, and/or hybrid) for assigned A-MDTs, including taking meeting notes
Provides case consultations to professionals in the community working on cases of adult abuse (age 18 to 59), no direct client intervention
Coordinates the intake, triage, and presentation of adult abuse cases for A-MDT review
Facilitates/Communicates action plans, case follow-ups, and responses to emerging case issues in a timely manner
Tracks and reports data from A-MDT discussions, consultations, and outreach
Participates in regularly scheduled activities such as technical assistance and workgroups, as assigned
Presents at local, statewide, and national conferences, trainings and seminars as needed with the approval of the Director for E-MDT Initiatives and within the financial resources and allowable expenses of the program
Maintains up-to-date knowledge and skills by attending ongoing conferences, trainings, and seminars with the approval of the Director for E-MDT Initiatives and within the financial resources of the program
Other duties as assigned by UpEAC Division Leader or Director for E-MDT Initiatives
Qualification
Required
Bachelor's Degree in Social Work, or related field
A minimum of two years' experience in Human Services
Strong working knowledge of community resources and organizations required
Ability to work as part of a team and collaborate with a wide range of professionals, including law enforcement, social services, medical and mental health providers
Ability to travel by car
Must have reliable transportation and valid driver's license to travel within the service region
At least half of work is performed at a desk or at a personal computer station
Comfort with virtual collaboration and ability to build and maintain relationships remotely
Excellent verbal, written and telephone communication skills are required
Strong written and computer skills (MS Office) needed
Ability to work independently
Ability to maintain composure under strict deadlines and difficult situations
Preferred
Master's Degree in Social Work preferred
Prior experience in adult services, mental health, or a closely related field, preferred
