Aging and Disability Resource Center (ADRC) Specialist
The State of Washington is seeking an Aging and Disability Resource Center (ADRC) Specialist for the Olympic Area Agency on Aging (O3A) based in Aberdeen, WA. The ADRC Specialist will provide information and resources to older adults and individuals with disabilities, conduct eligibility screenings, and deliver case management services.

Responsibilities
Provides accurate and relevant information to individuals seeking resources including older adults, family caregivers, adults with disabilities and Medicaid/WA Cares beneficiaries
Conducts program and service eligibility screening and needs assessments
Delivers services and options counseling under targeted community-based programs, including: Care Transitions, Health Related Social Needs (HRSN), Medicaid access, and WA Cares
The ADRC Specialist provides individualized and comprehensive short-term case management services
Operates with a significant level of independence, requiring minimal supervision
Requires exemplary customer service skills
Qualification
Required
Requires a Bachelor's Degree in behavioral, health sciences, or related field with two years' experience providing direct human services; or MA in behavioral, health sciences or related field with one year experience providing direct human service; or equivalent combination of education and experience
Requires current WDL & insured vehicle
