Completing and coordinating timely MDS assessments and submissions
Collaborating with the interdisciplinary team on resident care plans
Ensuring documentation meets regulatory and quality standards
Tracking resident changes and updating care plans accordingly
Supporting audit readiness, case-mix accuracy, and survey prep
Qualification
Required
Valid NYS LPN license
Strong attention to detail and clinical judgment
Excellent communication and teamwork skills
Preferred
Experience with MDS
Prior experience in skilled nursing or long-term care
