Responsibilities

Oversee day-to-day operations for CHAP’s OCMH and Healthy Families America (HFA) programs
Monitor program deliverables, timelines, and quality standards to ensure we stay on track with grant expectations
Support Coordinators in problem-solving barriers and improving workflows
Collaborate with supervisors and CHWs to strengthen enrollment, documentation, and participant engagement
Ensure accurate, timely data entry and document management across all CHAP programs
Review data for trends, quality issues, and compliance risks
Prepare monthly, quarterly, and annual reports for OCMH, HFA, PCHI, and other funders as needed
Maintain strong working knowledge of CCS and other data platforms used by CHAP
Directly supervise Program & Data Coordinators, including regular check-ins, performance development, and coaching
Provide guidance on program expectations, documentation needs, and workflow changes
Support training needs and help onboard new staff connected to program operations
Lead with a supportive, solutions-focused approach consistent with a Just Culture environment
Partner with the Executive Director and Operations & Training Manager to support quality improvement efforts
Help maintain policies, procedures, and program manuals related to OCMH, HFA, and broader HUB functions
Ensure CHAP meets requirements tied to fidelity, confidentiality, and data security
Support communication with funders, evaluators, community partners, and referral sources
Participate in meetings, site visits, and audits for OCMH, HFA, and other programs
Strengthen CHAP’s relationships with healthcare and community partners through consistent follow-up and professional communication

Required

Bachelor's degree in public health, social work, human services, or a related field (or equivalent experience)
Experience in program management, data systems, or grant compliance preferred
Strong understanding of community health workers and the Pathways Community HUB model is a plus
Excellent organizational skills and comfort managing multiple priorities at once
Strong communication skills and the ability to lead, coach, and support staff
