Responsibilities

Oversee program operations, ensuring compliance with NYS DOH, MCO, and organizational standards
Supervise and develop Care Management Supervisors and staff to foster excellence in care coordination
Manage program enrollment, outreach, and reporting to support census accuracy and service delivery goals
Lead quality improvement efforts and use performance data to enhance program efficiency and outcomes
Collaborate across departments to ensure integrated, patient-centered care
Partner with Finance and Billing to support reimbursement accuracy and fiscal performance
Maintain audit readiness and ensure documentation meets regulatory and compliance standards
Strengthen community and hospital partnerships to support referrals and member retention

Required

Masters in Social Work, Psychology, Nursing, Public Health, or related field required
Minimum 5 years of experience in care management, case management, or care coordination within healthcare, behavioral health, or social service settings
Minimum 3 years of supervisory or leadership experience overseeing multidisciplinary teams
Strong understanding of NYS Health Homes regulations, managed care, and integrated service delivery
Excellent interpersonal, communication, and analytical skills
Proficiency in Microsoft Office Suite, EHR systems, and care coordination platforms
