Responsibilities
Ensure timely, professional, and coordinated responses to both referral sources and prospective clients
Maintain accurate and comprehensive records of all referral activity
Collaborate with regional teams to enhance patient/client advocacy and streamline case management processes
Qualification
Required
Experience with homecare referral systems and homecare contracting in Washington State
Knowledge of Long-Term Services and Supports (LTSS) and community-based care programs
Associate degree in healthcare administration, social services, or related field
Two (2) years of experience in healthcare, homecare, or case management intake coordination
Preferred
Bachelor's degree in healthcare administration, social services, or related field
Two (2) years of experience in healthcare or homecare or case management intake coordination
