New Grad, Entry LevelRichmond Behavioral Health is seeking full-time Support Coordinators to join their Adult Developmental Services Team, who will coordinate and monitor services for adults with intellectual disabilities and developmental disabilities. The role involves completing assessments, developing person-centered service plans, linking to service providers, and ensuring compliance with regulatory requirements.

Responsibilities
Assesses client service needs, capabilities and appropriateness for services; presents options and services based upon a needs assessment and client preference
Develops a person centered plan of care and services appropriate to the individual; reviews and updates service plans
Makes referrals and linkages to appropriate agencies for services; coordinates client services and treatment with multiple service providers and agencies
Evaluates the quality of services provided and monitors the client's well-being; evaluates client's environment for safety
Serves as liaison to public agencies and provides information regarding Authority programs and services
Provides crisis intervention and prevention services
Prepares a variety of reports; prepares and maintains client charts and other records
Attends meetings, staffing's, and conferences as they relate to client, staff and program needs
Qualification
Required
Full-time availability
Experience in coordinating and monitoring services for adults with intellectual disabilities and developmental disabilities
Ability to complete assessments and develop person-centered service plans
Skills in linking to service providers and monitoring service plans
Advocacy for client preferences and service needs
Ensuring documentation complies with regulatory, reimbursement, and agency requirements
Ability to assess client service needs, capabilities, and appropriateness for services
Experience in making referrals and linkages to appropriate agencies for services
Coordination of client services and treatment with multiple service providers and agencies
Evaluation of the quality of services provided and monitoring client's well-being
Evaluation of client's environment for safety
Ability to serve as a liaison to public agencies and provide information regarding Authority programs and services
Experience in providing crisis intervention and prevention services
Ability to prepare a variety of reports and maintain client charts and other records
Willingness to attend meetings, staffing's, and conferences as they relate to client, staff, and program needs
