Responsibilities

Identify, resolve, and prevent medication-related problems to ensure safe and effective pharmacotherapy

Conduct comprehensive assessments for chronic disease management under collaborative practice agreements or clinic protocols

Stratify patients based on clinical risk to prioritize interventions and tailor care plans accordingly

Provide individualized counseling on medication use, lifestyle modifications, and disease state education

Address barriers to adherence, including cost, access, and health literacy, through targeted interventions

Monitor and evaluate medication regimens to ensure clinical criteria are met, including therapeutic goals, adverse effects, and drug interactions

Work on-site in close coordination with interdisciplinary teams, including both in-person and hybrid care models

Ensure all clinical criteria are in clinical compliance with guidance from FDA, clinical guidelines, and appropriate evidence-based literature

Willing to do whatever is necessary for the betterment of the organization

Protects organization's value by keeping information confidential
