Responsibilities

Perform rounding to collect discharged patient charts and reconcile with discharge logs

Prep, scan, index, and quality-check medical records into the EMR system in a timely and accurate manner

Analyze medical records for completion and assign/reassign provider deficiencies per facility policy and regulatory standards

Handle and process requests for patient medical records in accordance with HIPAA regulations

Assist physicians with record completion processes and monitor for compliance with documentation timeliness

Retrieve, sort, and manage patient medical records and related correspondence

Maintain logs, track deficiencies, and generate reports as directed

Provide support for chart assembly, data abstraction, and HIM-related audits

Participate in department meetings and performance improvement initiatives

May serve as primary or backup for specific HIM functions as designated by management
