Lead, Care Coordinator, Transitional Care Management- Eastern Time Zone

Responsibilities
Supervises daily operations of market Care Coordinator staff under the direction of Market Operations leaders
Participates in employee performance review process
Participates in interviewing, hiring, onboarding, and precepting new Care Coordinator staff for the market
Helps conduct chart audits per established policy, ensures training programs address identified needs of staff, and operates as market subject matter expert on care coordinator workflows
Performs outbound calls to patients to understand their clinical needs and connect them with appropriate resources
Performs outbound calls to providers to make appointments for patients or follow up on care
Answers inbound calls from patients, providers, and other resources
Coordinates with clinical resources and providers to ensure smooth continuum of care for patients
Assists with completing applications for resources, paperwork for provider visits, etc
Provides patients with education materials and sends communications to primary care physicians, nephrologists, and specialists for new enrollments/appointments
Works with leadership to support CC team in meeting the organizational goals of the market
May be a preceptor in their markets or assist with the scheduling, interviewing, and performance management of the preceptorship program
Qualification
Required
6+ years combined of related education, experience, or certification
Current BLS or CPR Certification required
Efficient and reliable transportation, including an active driver's license, allowing for the ability to travel across an assigned region to meet patient needs. Locations may include offices, clinics, and patient homes
Provides in-person patient care which may include standing, sitting, walking, pushing, pulling, and lifting
Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms
Preferred
Customer service experience
Current, unrestricted license as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state(s) of practice with current community health worker experience
Community Health Worker (CHW) Certification
Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook
