Responsibilities

You will perform assessment and evaluation of potential care management patients per the client's contractual obligations and/or state requirements

You will collaborate with health care providers and ancillary service providers to assure coordination of care to ensure effectiveness of treatment plans and identify educational opportunities

You will provide ongoing education, resource information, and support to patients that reinforce their treatment plan and enables them to improve their health management skills

You will evaluate treatment adherence, member safety, and socioeconomic factors, and re-evaluate the patient’s commitment to making the necessary changes on a regular basis

You will document per contractual requirements related to individual member status, and/or medical necessity review, and negotiation
