ACT Team Specialist

They are seeking an ACT Team Specialist to provide assertive community treatment services to clients with severe mental illness, assisting them in daily living skills and connecting them to necessary resources.

Responsibilities
Provides assertive community treatment services to clients assigned to the ACT Program in accordance with the State mandates and established Agency and program policies, procedures, philosophy and client individual service plan
Maintains client confidentiality and ensures release of information is handled in accordance with federal guidelines and agency policy/procedures
Qualification
Required
Bachelor's degree OR a QMHS 3+ with high school diploma required
At least one year experience working with SPMI clients in a community setting
Must demonstrate knowledge of mental illness and the ability to establish and maintain therapeutic relationships with clients
Preferred
Prior ACT experience preferred
LSW or LPC preferred
Must obtain CPR and CPI certification following hire
If not licensed, must meet Ohio MHAS criteria for qualified mental health specialist (QMHS)
