Deborah Heart and Lung Center is seeking an Assistant Advanced Heart Failure Program VAD Coordinator to oversee the operations of their Advanced Heart Failure Program. The role involves direct patient contact, care coordination, and collaboration with a multidisciplinary team to ensure optimal patient care and efficiency in managing heart failure procedures.

Responsibilities

Function as a central figure in the overall operations of the Advanced Heart Failure Program including CHF, LVAD, and other related procedures

Direct patient contact and coordination of care

Assist the AHF providers and LVAD Coordinator with weekly meetings, presentations, and any other tasks that they deem necessary

Schedule outpatient evaluations and follow-ups

Provide patient education and maintain the ELSO registry

Work with a multidisciplinary team to ensure optimal patient care and to maximize efficiency in managing the heart failure program

Qualification

Required

2-3 years experience in cardiovascular nursing required

Minimum Registered nurse RN required

Current license in NJ as RN

Current BLS required

Proficiency with electronic health records and database management required

Excellent written and verbal communication skills

Proficient with Microsoft Office Suite including WORD, EXCEL, and POWERPOINT

Well organized, detail oriented, multi-tasking

Preferred

1+ years experience in Heart Failure preferred
