Responsibilities

Manage the development and implementation of health care improvement initiatives related to the MPT - 1115 waiver and coordination with the Indian health care delivery system including Tribal, Urban and Indian Health Services in Washington State
Coordinate, provide consultation to, and work in partnership within the Office of Tribal Affairs(OTA), Health Care Authority (HCA), across DOC divisions, with Tribes, and Native-led, Native-serving organizations plus other state agencies and interested parties, such as Accountable Communities of Health (ACH) to advance and organize initiatives/resources relating to the MTP - 1115 waiver and the agency’s implementation of MTP to support successful transition for eligible individuals
Using independent judgment, produce, edit, research, analyze, and provide constructive analysis to management, staff and partners regarding complex issues impacting the 1115waiver, on documents such as communication materials, contracts, and those required by the Health Care Authority (HCA). Working with the MTP Operations Manager to provide implementation plans and quarterly progress reports related to the 1115 waiver and AI/ANs
Attend and participate in internal and external meetings pertaining to the 1115 waiver
Develop recommendations and implement approved strategic processes for monitoring the implementation of the MTP - 1115 waiver initiatives and evaluation of the specific program policies and procedures for effectiveness in relation to tribal healthcare
Provide education and/or clarification via presentations or constructive feedback to others in understanding the Indian health care system and how the 1115 waiver could be used to support and/or harm Indigenous (Tribal) communities
Coordinate with other MTP leads and Subject Matter Experts from HCA, DOC and other state agencies as directed by supervisor to ensure communications and overall strategic and long-range planning activities are aligned
Identify problems; analyzes and evaluates process and operating deficiencies or difficulties; provide MTP Operations Manager with alternatives and recommended courses of action
Providing timely reporting and documentation to reflect growth and development of agency tribal health program; capture data that identifies service populations and where they release to and with what resources
Anticipate, frame, and pose questions to appropriate team members for critical policy analysis and evaluate results to determine validity
Attend other trainings as directed or approved by supervisor
Coordinate the development of partnerships and resources that support individualized reentry plans and health services reentry plans for Washington State tribal members releasing from DOC confinement
Working with DOC Grants Manager to research and apply for Indian Health-related opportunities that support reentry or other supportive opportunities
Establishing necessary contracts/MOUs to improve continuity of care with tribal communities
Working with MTP Operations Manager and coordinating with HCA staff to expand future waiver benefits to include traditional healing options, where approved and supported by CMS, HCA, and local leadership
Develop continuity of care for Native Veterans, connecting to tribal health care, Tribal VA Coordinators and health-related social needs (HRSN)
Participate and provide consultation/ support to individuals releasing from prison to community or partial confinement through individual or multidisciplinary team meetings for eligible individuals
Partner closely with facility-based Health Services Reentry Teams to ensure AI/AN individuals
Partnering with MTP Operations Manager to create sustainable data gathering for measures and reporting needs
Managing assigned projects and project timelines, update of electronic database and information sharing platforms for transparency and tracking purposes
Connecting strategies and developing continuous improvement methods in support of the DOC Health Plan and Health Services policies
Coordinating adequate resource deployment to achieve objectives and outcomes within planned timeframes, adjusting plans as needed for capacity
Administering agency and divisional portfolios and providing direction in the development of master project plans, charter, schedule, budget, and reporting
Monitor and control master project plans, report status, lead risk mitigation, resolve issues, and adjust plans as required
Providing professional level coaching to develop and support individual and group skills, proficiency and effectiveness for team members, process owners, and sponsors
Communicating and reinforcing organizational culture to value participants and achieve results
Participate in quality improvement activities and projects as assigned

Required

At least five (5) years of experience working with tribal governments, tribal health programs, Indian health service, or urban Indian health programs, including applying nation to nation communication protocols, sovereignty principles, or tribal consultation practices
At least two (2) years of experience interpreting, analyzing, or applying print policies, laws, rules, regulations, or intergovernmental agreements, and translating them into operational guidance
Experience supporting individuals navigating health systems, tribal health pathways, or reentry related services, with the ability to apply knowledge of IHS, tribal 638 programs, or urban Indian health programs
Demonstrated ability to communicate complex information clearly in writing and verbally to diverse audiences, including tribal leaders, DOC staff, and community partners
Experience coordinating projects, casework, or cross system activities involving multiple agencies, jurisdictions, or sovereign governments
The ability to apply an equity lens to decision making communication, and service delivery, consistent with Washington's enterprise DEI competencies

Preferred

Experience working within or in partnership with tribal health programs, IHS, or urban Indian health programs
Understanding of healthcare as a reserved right and the federal trust responsibility
Familiarity with Washington's Medicaid transformation project, 1115 waiver, or tribal specific Medicaid policies
Experience coordinating reentry services for American Indian/Alaska Native individuals or working with culturally specific reentry pathways
Experience facilitating meetings, workgroups, or consultations involving tribal, state, or federal partners
At least one year's experience in a supervisory, lead, or administrative capacity
Bachelor's degree or higher in a related field such as public administration, public health, social work, health administration, tribal governance and administration, law, business administration, or another field supporting the competencies of the role
