APP - Program Coordinator RPM
LifeBridge Health is seeking a dynamic Program Coordinator for Remote Patient Monitoring (RPM) to oversee the implementation and daily operations of the RPM program. This role ensures clinical effectiveness and operational efficiency while providing telemedicine care and leading RPM program deployment across various entities.

Responsibilities
Leadership & Deployment: Lead the deployment of the RPM program across all LifeBridge Health entities, ensuring successful integration and growth
Collaboration: Work closely with stakeholders, population health teams, and virtual care teams to ensure alignment with program goals
Platform Support & Vendor Relations: Collaborate with IT teams and vendors to configure and support the RPM platform
Performance Monitoring: Track program Key Performance Indicators (KPIs) and report outcomes to leadership
Compliance: Ensure the program meets all relevant standards, including CMS, HIPAA, and internal LifeBridge policies
Telemedicine Care: Provide remote patient care through telemedicine platforms in collaboration with physicians and other healthcare professionals
Comprehensive Assessments: Conduct detailed medical assessments, including health history and diagnostic tests, to contribute to diagnosis and treatment plans
Treatment & Care Plan: Work with care teams to initiate treatment plans and perform clinical assessments for both routine and complex patient cases
Documentation & Orders: Perform necessary clinical documentation and tasks, such as order initiation, medication reconciliation, and discharge paperwork
Collaboration: Serve as a clinical resource for nursing staff and other team members, escalating findings to the Clinical Director as appropriate
Medication Management: Prescribe medications in accordance with federal, state, and institutional regulations, and order necessary diagnostic tests
Patient Education: Develop and provide patient education materials promoting current clinical practices and guidelines
Teamwork: Foster a positive working relationship with patients, families, co-workers, and leadership to promote teamwork and a collaborative work environment
Qualification
Required
Minimum of 7 years of healthcare experience, with a strong background in telemedicine or remote patient monitoring
Master's degree in Nursing (MSN), Physician Assistant Studies (MPAS), or Nurse Practitioner (NP) required
Current Maryland Physician Assistant (PA) or Nurse Practitioner (CRNP) license required
Strong knowledge of remote patient monitoring technologies and telemedicine platforms
Excellent clinical assessment skills with the ability to manage both routine and complex patient cases remotely
Strong leadership and organizational skills, with the ability to collaborate effectively across multidisciplinary teams
Familiarity with CMS, HIPAA, and telemedicine regulations and guidelines
Exceptional communication skills (both verbal and written), with the ability to effectively interact with patients, families, and healthcare teams
Proficiency with electronic medical records (EMR) systems and other healthcare technologies
Preferred
Proven leadership experience in program coordination or management is highly preferred
