TapestryHealth is seeking a Weekend DQR Team Lead Clinical Data Reviewer to ensure the delivery of high-quality patient reports for their Care Management service line. The role involves reviewing patient medical data from EMR systems and compiling monthly Chronic Care Management reports while adhering to established protocols and standards.

Responsibilities

Responsible for pulling and compiling key patient medical data from external systems and entering it into Chronic Care Management system

Review monthly summary reports on patients’ health status using collected data, highlighting any significant changes or abnormalities for APPR clinical review and treatment planning

Adhere to all established department policies, protocols, and workflows

Meet established standards for productivity as assigned

Help identify system issues and communicate that to the manager

Work with Tapestry IT to resolve discrepancies with patient data

Protect patient information and adhere to confidentiality standards

Contribute and participate in continuous quality improvement activities

Communicate effectively with 1099 DQR team members

Provide facility assignments as needed

Review DQR charts for accuracy and completeness

All other duties as assigned

Work scheduled weekends as assigned

Required

Current and valid Medical Assistant license or Certified Nursing Assistant or related fields

Minimum of 3 years' experience as a Medical Assistant or Certified Nursing Assistant or related fields

Proficiency in electronic health records systems

Proficient with Microsoft Office Suite, and Dashboard type platforms

Familiarity with HIPAA regulations and patient privacy standards

Preferred

Current and valid LPN license

Minimum two years of college in related field

Strong verbal/written communication

Ability to organize and prioritize tasks

Ability to work independently, be attentive to detail and maintain a positive attitude

Ability to educate internal staff on care management services
