This role involves advocating for older adults by providing comprehensive care management services, including conducting assessments, making home visits, and coordinating with other service providers to assist clients in obtaining necessary services.

Responsibilities

Provide care management services to older adults throughout Northumberland County

Obtain information from clients, family members, neighbors, and others to identify social, economic, emotional, health, or physical problems

Assist clients in obtaining a variety of services in accordance with proper procedures and established policies

Provide information and referrals to consumers and their families

Make regular home visits and counsel consumers

Participate in formal and informal training sessions

Qualification

Required

Six months as an Aging Care Manager 1 or a County Caseworker 1; or

Successful completion of the County Social Casework Intern program; or

Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or

An equivalent combination of experience and training

You must meet the PA residency requirement

You must be able to perform essential job functions

This position falls under the provisions of the Older Adult Protective Services Act

Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance
