The primary function is the provision of direct patient care, including collaboration with other team members and participation in the development of the patient’s comprehensive plan of care
Rehabilitation equipment including but not limited to: splints, orthoses, prostheses, wheelchairs, ADL equipment, Modalities
Candidate required to be Home Health Certified
Qualification
Required
2+ acute nursing care (hospital base) and Home Care experience
Valid Driver License
Candidate required to be Home Health Certified
Certified home health experience including proficiency with OASIS required and NON-NEGOTIABLE
